To report a cemetery in crisis, include such information as:
- Name of Cemetery
- Cemetery Number, if known - click here for list
- Name of owner, if known
- Location (be specific)
- Conditions observed, e.g.: overgrown; trash present; markers vandalized, broken or tipped over; fence broken or missing; threatened by construction
To request a speaker for an event, provide the following:
- Organization Name
- Meeting Location
- Date and Time of Event
- Length of desired program
- Type of audience, e.g.: civic association; scouts; seniors; students
- Audience size
- Contact person: name, email, telephone numbers
